About your visit

What To Expect:
Please see our home page to locate directions to the office,how to prepare for your visit, and instructions on how to register on Medem for services such as making appointments online, requesting prescription refills online, making appointments online, online physician consultation (a charge applies) and completing your medical history and symptom history before your visit using Instant Medical History.

Preprocedure:
Before your visit, please verify your insurance status and membership numbers with your insurance company or employer. Please bring any co-pays (a charge for billing for co-pays not paid at the visit may apply). Bring all prescription bottles, inhalers, nonprescription medication bottles, immunization and any other records that you have. Please also bring the name, phone and fax numbers of any physicians from whom you may wish us to request records. If you have a regular pharmacy, please bring their phone and fax number. If you bring your prescription plan formulary,we will be more easily able to avoid medication payment rejections by your insurer.

Postprocedure:
If you need referrals, please call, email or fax the office with the name, phone and fax number, and insurance provider number of any physicians or facilities for whom you need referrals. Please allow 2 business work days for referrals. Most referral can be done online.
Usually, we give prescriptions until you are due for a check-up or follow-up blood test. Please bring medicine bottles to your appointments so that we can make sure you have enough until your next appointment. Please allow 2 business days for any prescription refill requests.


Staff Information

Our staff is caring and focused on giving you the best care possible. Phaedra Johnson schedules appointments, takes messages,makes referrals. Andrea Williams prepares patients to see the doctors, draws blood and gives immunizations. She also helps with billing problems related to laboratory tests. Michelle Harper and Sabrina Wise provide these services on Saturday mornings. Margaret Lytton, MD, Maria Warnick, CRNP and Christine Stallkamp, MD, are the primary healthcare providers.For questions related to a bill you have received, please call Mediclaims at 800-572-0809.
